Alabama
Through this SPA, Alabama is permitted to implement a health services initiative (HSI) to provide prenatal monitoring, case management and care coordination services to low-income, high-risk pregnant women and their infants in Montgomery, Macon, and Russell counties in order to improve pregnancy outcomes and infant health. Patients will receive services during pregnancy and for up to one year post delivery.
