Medicaid Data Collection Tool (MDCT) Portal

The Centers for Medicare and Medicaid Services (CMS) collect data from United States and territories to better understand the Medicaid and Children’s Health Insurance Program (CHIP) programs.

The Center for Medicaid and CHIP Services (CMCS) has developed the Medicaid Data Collection Tool (MDCT) and underlying applications to collect the data required to best monitor, manage, understand and review the Medicaid and CHIP programs for each state.

CHIP Annual Reporting Template System (CARTS)

CARTS can be accessed here.

CARTS allows states to report data to CMS regarding the coverage of their CHIP state plan on an annual basis. For CARTS assistance, please email the help desk at MDCT_Help@cms.hhs.gov.

Statistical Enrollment Data System (SEDS)

SEDS can be accessed here.

SEDS collects CHIP and Medicaid quarterly enrollment data from states on a quarterly basis. For SEDS assistance, please email the help desk at MDCT_Help@cms.hhs.gov.

Quality Measures Reporting (QMR)

QMR can be accessed here.

QMR collects state data to measure or quantify healthcare processes to ensure quality health care for Medicaid Beneficiaries. For QMR assistance, please email the help desk at MDCT_Help@cms.hhs.gov.
