The Centers for Medicare & Medicaid Services (CMS) Testing Experience and Functional Tools (TEFT) demonstration program provided funding to grantee states to implement personal health record (PHR) pilots specifically focused on implementing a PHR for beneficiaries of Medicaid home and community-based services (HCBS) programs. Through the PHRs, CMS and states aimed to demonstrate the use of PHRs containing medical and social service information.
- Evaluation Tools for Personal Health Record Initiatives in Medicaid Home and Community-Based Services Programs: This report describes the PHR pilots that were conducted under the TEFT demonstration and planning and evaluation tools that were used as part of the pilots.
- Grantee Experience with PHR Stakeholder Engagement (June 2015): This report describes four TEFT grantee strategies to obtain and maintain stakeholder involvement in developing requirements of the PHR system.
