Personal Health Record

The Centers for Medicare & Medicaid Services (CMS) Testing Experience and Functional Tools (TEFT) demonstration program provided funding to grantee states to implement personal health record (PHR) pilots specifically focused on implementing a PHR for beneficiaries of Medicaid home and community-based services (HCBS) programs. Through the PHRs, CMS and states aimed to demonstrate the use of PHRs containing medical and social service information.

Evaluation Tools for Personal Health Record Initiatives in Medicaid Home and Community-Based Services Programs: This report describes the PHR pilots that were conducted under the TEFT demonstration and planning and evaluation tools that were used as part of the pilots.
Grantee Experience with PHR Stakeholder Engagement (June 2015): This report describes four TEFT grantee strategies to obtain and maintain stakeholder involvement in developing requirements of the PHR system.
