In addition to one-on-one technical assistance, the Centers for Medicare & Medicaid Services (CMS) offers states that are pursuing health homes for Medicaid beneficiaries with chronic conditions opportunities for collaborative learning.
The Peer Learning Network provides a forum for states to share health home models, elements of their state plan amendments, and successes/challenges in their development processes. Call topic are based on feedback from state participants on areas of interest. The Peer Learning Network allows dissemination of learnings, tools, and resources among peers and provides networking opportunities for participating states. Calls are open to state staff whose Medicaid agencies are designing or implementing a health home model.
