WAIVER TERMINATED Effective 10/1/2013 – Provided case management, community integrated employment, day habilitation, homemaker, personal care, prevocational services-phased out year 1, residential habilitation, respite, supported living, OT, PT, speech therapy, agency with choice, independent support broker, companion services, dietician services, environmental mods, in home support-phased out year 1, individually-directed goods and services, skilled nursing, specialized equipment, unpaid caregiver training and education for individuals with intellectual disabilities and developmental disabilities ages 21 to no max age.
WAIVER TERMINATED Effective 9/1/2016 – Provided case management, assisted living service level I, assisted living service level II, and assisted living service level III for aged individuals ages 65 to no max age and individuals with physical disabilities ages 19 to 64 years.

WAIVER TERMINATED Effective 10/1/2013 – Provided case management, community integrated employment, homemaker, personal care, residential habilitation, respite, special family habilitation home, agency with choice, independent support broker, child habilitation services, companion services, dietician services, environmental mods, individually-directed goods and services, residential habilitation training, skilled nursing, specialized equipment, supported living, unpaid caregiver training and education for individuals with intellectual disabilities and developmental disabilities ages 0 to 20 years.

Provides youth and family training and support services for individuals with mental illness ages 18 to 21 and for individuals with serious emotional disturbance ages 4 to 17 years.

Provides adult day services, case management, home health aide, personal support services, respite, skilled nursing, assisted living facility services, home delivered meals, non-medical transportation, and personal emergency response system (PERS) for aged individuals 65 – no max age and individuals with other disabilities ages 19-64.
