Provides group day services, independent living supports, individual supported employment, assistive technology, benefits planning, center-based crisis supports, community coaching, community engagement, community guide, community-based crisis supports, crisis support services, electronic home-based supports, employment and community transportation, environmental modifications, group supported employment, peer mentor supports, personal emergency response system, shared living, and transition services to individuals with autism, intellectual disabilities, or developmental disabilities ages 18 or older who meet an ICF/IID level of care.

Provides adult day health care, personal assistance services, respite care, services facilitation, assistive technology, environmental modifications, personal emergency response system, private duty nursing, and transition services to individuals ages 65 or older, individuals with physical or other disabilities ages 0-64 years, and individuals who are technology dependent ages 0 or older who meet a hospital or nursing facility level of care. This waiver operates with a concurrent 1915(b)(1) and 1915(b)(4).

Provides group day services, group home residential, individual supported employment, personal assistance services, respite, consumer-directed services facilitation, assistive technology, benefits planning, center-based crisis supports, community coaching, community engagement, community guide, community-based crisis supports, companion services, crisis support services, electronic home-based services, employment and community transportation, environmental modifications, group supported employment, in-home support services, peer mentor supports, personal emergency response system, private duty nursing, shared living, skilled nursing, sponsored residential, supported living, therapeutic consultation, transition, and workplace assistance services to individuals with autism, intellectual disabilities, or developmental disabilities ages 0 or older who meet an ICF/IID level of care.

Provides group day services, in-home support services, individual supported employment, personal assistance services, respite, services facilitation, assistive technology, benefits planning, center-based crisis supports, community coaching, community engagement, community guide, community-based crisis supports, companion services, crisis support services, electronic home-based supports, employment and community transportation, environmental modifications, group supported employment, individual and family/caregiver training, peer mentor supports, personal emergency response system, private duty nursing, shared living, skilled nursing, supported living residential, therapeutic consultation, workplace assistance, and transition services to individuals with autism, intellectual disabilities, or developmental disabilities ages 0 or older who meet an ICF/IID level of care.

WAIVER TERMINATED on 6/30/2018 – Provided assisted living services to individuals ages 65 or older and individuals with other disabilities ages 18-64 years who met a nursing facility level of care.
WAIVER TERMINATED on 9/30/2017 – Provided in-home residential supports, respite, service facilitation, companion, environmental mods (home accessibility adaptations), family/caregiver training (training and counseling services for unpaid caregivers), therapeutic consultation (clinical and therapeutic services), and transition coordination services to individuals with serious emotional disturbance ages 0-21 years who met a psychiatric residential treatment facility level of care.
WAIVER TERMINATED on 6/30/2017 – Provided personal care, respite (skilled private duty nursing), assistive technology, environmental modifications, private duty nursing, and transition services to individuals who are technology dependent ages 0 or older who met a hospital or nursing facility level of care.
WAIVER TERMINATED on 6/30/2012 – Provided case management, personal assistance, respite care, enteral nutrition, consumer directed services facilitation, assistive technology, environmental mods, personal emergency response system, private duty nursing, and transition services to individuals with HIV/AIDS ages 0 or older who met a hospital or nursing facility level of care.
