Provides adult day health care, case management, personal care, respite- institutional, adult day health care transportation, adult day health care-nursing, attendant care, companion care- agency, companion care-individual, home accessibility adaptations- environmental modifications, home delivered meals, personal emergency response system- installation, personal emergency response system- monitoring, pest control, residential personal care II, respite- CRCF, respite- in-home, specialized medical equipment and supplies, and telemonitoring services to individuals ages 65 or older and individuals with physical disabilities ages 18-64 years who meet a nursing facility level of care.  This waiver operates with a concurrent 1915(a)(1)(a) authority.

Provides adult day health care services, personal care services, respite care services, waiver case management, incontinence supplies, adult day health care nursing, assistive technology and appliances assessment/consultation, assistive technology and appliances, behavior support services, career preparation services, community services, day activity, employment services, environmental modifications, in-home support services, personal emergency response systems, private vehicle assessment/consultation, private vehicle modifications, and support center services to individuals with intellectual disabilities ages 0 or older who meet an ICF/IID level of care.
Provides attendant care/personal assistance services, career preparation services, day activity, residential habilitation, respite care, waiver case management, incontinence supplies, nursing services, occupational therapy, physical therapy, speech and hearing services, peer guidance for participant-directed care, behavioral support services, employment services, environmental modifications, health education for participant-directed care, independent living skills, personal emergency response systems, pest control bed bugs, pest control treatment, private vehicle assessment/consultation, private vehicle modifications, psychological services, specialized medical equipment/supplies/assistive technology - (assessment/consultation), and specialized medical equipment/supplies/assistive technology services to individuals with physical or other disabilities, or brain injury ages 0 or older who meet a nursing facility or ICF/IID level of care.

Provides case management, personal care, attendant care, companion care-agency, companion care- individual, home accessibility adaptations- environmental modifications, home delivered meals, nursing, pest control, and specialized medical equipment and supplies to individuals with HIV/AIDS ages 0 or older who meet a hospital level of care.  This waiver operates with a concurrent 1915(a)(1)(a) authority.

Provides adult day health care services, personal care, residential habilitation, respite care, waiver case management, adult dental services, adult vision, audiology services, incontinence supplies, adult attendant care, adult companion services, adult day health care nursing, behavior support services, career preparation, community services, day activity, employment services, environmental modifications, independent living skills, personal emergency response system, pest control bed bugs, pest control treatment, private vehicle assessment/consultation, private vehicle modifications, specialized medical equipment and assistive technology assessment/consultation, specialized medical equipment/supplies and assistive technology, and support center services to individuals with intellectual disabilities ages 0 or older who meet an ICF/IID level of care.

Provides case management, personal care, respite-institutional, attendant care, home accessibility adaptations-environmental modifications, home delivered meals, nursing services, personal emergency response system (PERS) – installation, PERS – monitoring, pest control, respite-in home, and specialized medical equipment and supplies to individuals who are technology dependent ages 21 or older who meet a nursing facility level of care.  This waiver operates with a concurrent 1915(a)(1)(a) authority. 

Provides nurse care coordination, respite, environmental modifications, and pediatric medical day care services to individuals who are medically fragile ages 0-21 years who meet a hospital level of care.

Provides high fidelity wraparound, respite, and individual directed goods and services to individuals with serious emotional disturbance ages 0-21 years who meet a hospital level of care.  This waiver operates with a concurrent 1915(b)(4) authority.
WAIVER TERMINATED on 12/31/2017 – Provided case management, early intensive behavioral intervention (EIBI) assessment, EIBI plan implementation, EIBI program development and training, lead therapy, line therapy I, line therapy II, self-directed line therapy I, and self-directed line therapy II services to individuals with autism ages 3-10 years who met an ICF/IID level of care.
WAIVER TERMINATED on 9/30/2017 – Provided case management, prevocational services, respite, customized goods and services, intensive family services, medication monitoring and wellness education, peer support, and wraparound para-professional services to individuals with serious emotional disturbance ages 4-18 years who met a psychiatric residential treatment facility level of care.
