Provides adult day health, case management, personal care, respite, extended state plan skilled nursing, prescribed drugs, advanced supportive/restorative assistance, assisted living services, assistive technology, consumer-directed personal assistance supports and services, environmental accessibility modifications, home delivered meals, hospice care, institution transition services, nursing, personal emergency response systems, remote supports, specialized medical equipment and supplies, and therapy services to individuals ages 65 or older and individuals with physical or other disabilities ages 19-64 years who meet a nursing facility level of care.

Provides adult day health, habilitation training specialist services, homemaker, prevocational services, respite, supported employment, nursing, prescribed drugs, agency companion, audiology services, community transition services, daily living supports, dental services, environmental accessibility adaptations and architectural modification, extended duty nursing, extensive residential supports, family counseling, family training, group home, intensive personal support, nutrition services, occupational therapy services, optometry, physical therapy services, psychological services, remote support, respite daily, self-directed goods and services, specialized foster care (also known as specialized family home care), specialized medical supplies and assistive technology, speech therapy services, and transportation services to individuals with intellectual disabilities ages 3 or older who meet an ICF/IID level of care.

Provides adult day services, habilitation training specialist services, homemaker, prevocational services, respite, supported employment, prescribed drugs, audiology services, dental services, environmental accessibility adaptations and architectural modification, family counseling, family training, nutrition services, occupational therapy, physical therapy, psychological services, remote supports, respite daily, self-directed goods and services, specialized medical supplies and assistive technology, speech therapy, and transportation services to individuals with intellectual disabilities ages 18 or older who meet an ICF/IID level of care.

Provides habilitation training specialist services, prevocational services, respite, supported employment, environmental accessibility adaptations and architectural modification, family training, occupational and physical therapy, respite daily, self-directed goods and services, and specialized medical supplies and assistive technology services to individuals with intellectual disabilities ages 3-17 years who meet an ICF/IID level of care.
Provides case management, personal care, respite, prescribed drugs, advanced supportive/restorative assistance, environmental modifications, home delivered meals, hospice care, institutional transition case management, personal emergency response system, private duty nursing, self-directed goods and services, skilled nursing, specialized medical equipment and supplies, respiratory therapy, occupational therapy, physical therapy, speech therapy, and transitional case management services to individuals who are medically fragile or who are technology dependent ages 19 or older who meet a hospital or nursing facility level of care.
WAIVER TERMINATED on 12/1/2015 – Provided adult day health, case management, personal care, respite, prescription drugs, skilled nursing, advanced supportive/restorative assistance, agency companion, assisted living, assistive technology, audiology treatment & evaluation, dental services & treatment, environmental mods, family counseling, family training, home delivered meals, hospice care, independent living skills training, institutional transition, nutritional education services, personal emergency response system, private duty nursing, psychiatry, psychological services, self-directed goods and services, specialized medical equipment and supplies, occupational therapy, physical therapy, respiratory therapy, speech/language therapy, transportation, and vision services to include eye exam & glasses to individuals with physical disabilities ages 20-64 years who met a nursing facility level of care.
