NM Developmental Disabilities Waiver Program (0173.R07.00) 

Provides case management, community integrated employment, customized community supports, living supports, respite, nutritional counseling, occupational therapy for adults, physical therapy for adults, speech and language therapy for adults, supplemental dental care, adult nursing, assistive technology, behavioral support consultation, crisis support, customized in-home supports, environmental modifications, independent living transition service, non-medical transportation, preliminary risk screening and consultation, remote personal support technology, and socialization and sexuality education services to individuals with autism, intellectual disabilities, or developmental disabilities ages 0 or older who meet an ICF/IID level of care.

NM Medically Fragile Waiver (0223.R06.00)

Provides case management, customized community group supports, home health aide, respite, nutritional counseling, skilled therapy for adults, behavior support consultation, environmental modifications, individual directed goods and services, private duty nursing, specialized medical equipment and supplies, specialized therapies, and vehicle modification services to individuals who are medically fragile ages 0 or older who meet an ICF/IID level of care.

NM Mi Via - ICF/IDD Waiver (0448.R04.00)

Provides consultant services, customized community group supports, employment supports, home health aide services, homemaker/direct support services, respite, skilled therapy for adults, personal plan facilitation, behavior support consultation, community direct support, emergency response services, environmental modifications, in-home living supports, individual directed goods and services, nutritional counseling, private duty nursing for adults, specialized therapies, and transportation services to individuals who are medically fragile, individuals with autism, with intellectual disabilities, or with developmental disabilities ages 0 or older who meet an ICF/IID level of care.

WAIVER TERMINATED – NM AIDS Waiver (0161.R04.00)

WAIVER TERMINATED on 1/1/2014 – Provided case management, homemaker/personal care, and private duty nursing to individuals with HIV/AIDS ages 0 or older who met a nursing facility level of care.

WAIVER TERMINATED – NM Coordinated Long Term Services Waiver (0479.R00.00)

WAIVER TERMINATED on 12/31/2013 – Provided adult day health, respite, service coordination, skilled maintenance therapy, assisted living, community transition relocation specialists, community transition services, emergency response, environmental modifications, and private duty nursing to individuals ages 65 or older and individuals with physical disabilities ages 0-64 years who met a nursing facility level of care.  This waiver operated with a concurrent 1915(b)(1) and 1915(b)(4) authority.

WAIVER TERMINATED on 9/28/2025 - Provided community supports coordinator, customized community supports – individual, customized community supports – group, employment supports, personal care, assistive technology, behavior support consultation, environmental modifications, non-medical transportation, respite, and vehicle modification services to individuals with autism, developmental disabilities, or intellectual disabilities ages 0 or older who met an ICF/IID level of care.
