Provides respite, assistive technology, child day habilitation, community integration, day supports, environmental modification assessment, family and peer mentoring, family caregiver training, home modifications, homemaker, independent living, non-medical transportation, personal emergency response system (PERS), supported family living, and vehicle modification services for individuals with autism, intellectual disabilities, or developmental disabilities ages 0-21 years who meet an ICF/IID level of care.

Provides prevocational, residential habilitation, respite, supported employment – individual, adult day, assistive technology, behavioral in-home habilitation, child day habilitation, community integration, consultative assessment, day supports, environmental modification assessment, home modifications, homemaker, independent living, medical in-home habilitation, personal emergency response system, small group vocational support, supported employment - follow along, supported family living, therapeutic residential habilitation, transitional services, transportation, and vehicle modification services to individuals with autism, intellectual disabilities, or developmental disabilities ages 0 or older who meet an ICF/IID level of care.

Provides prevocational services, respite, supported employment – individual, adult day, assistive technology, community integration, consultative assessment, day supports, environmental modification assessment, home modifications, independent living, personal emergency response system, small group vocational support, supported employment - follow along, supported family living, transitional, transportation, and vehicle modification services to individuals with autism, intellectual disabilities, or developmental disabilities ages 21 or older who meet an ICF/IID level of care.

Provides adult day health services, personal care, respite care, assisted living service, assistive technology, chore, companion, extra care for children with disabilities, home again, home and vehicle modifications, home delivered meals, independence skills building, non-medical transportation, and personal emergency response system services to individuals ages 65 or older and individuals with physical disabilities ages 0-64 years who meet a nursing facility level of care.

Provides supported employment – individual, TBI adult day health, TBI personal care, TBI respite care, assistive technology, caregiver training, chore, community connections, home delivered meals, home modifications, non-medical transportation, personal emergency response system (PERS), supported employment - follow along, TBI companion, TBI supported residential living, and vehicle modification services to individuals with brain injury ages 18 or older who meet a nursing facility level of care.

WAIVER TERMINATED on 9/30/2014 – Provided early intensive behavioral intervention to individuals with autism ages 0-17 years who met an ICF/IID level of care.

WAIVER TERMINATED on 9/30/2017 – Provided day habilitation, group home residential habilitation, integrated community employment, respite, assistive technology and supports, behavioral risk services, community living and day supports, companion home residential habilitation, extended family home residential habilitation, home modifications, in- home residential habilitation, medical risk services, personal emergency response system, retirement services, team behavioral consultation, vehicle modification, vocational planning habilitation, and workstation habilitation services to individuals with autism, intellectual disabilities, or developmental disabilities ages 21 or older who met an ICF/IID level of care.
