Provides adult residential for care for participants with acquired traumatic brain injury, and assisted living services to individuals ages 65 or older, and individuals with physical or other disabilities ages 21-64 years who meet a nursing facility level of care.

Provides adult day care, case management, in-home respite, personal care service, extended home health services, community transition services, environmental safety service, home delivered meals, institutional respite care, medication management, physical therapy, and speech therapy services to individuals ages 65 or older and individuals with physical disabilities ages 21-64 years who meet a nursing facility level of care.  This waiver operates with a concurrent 1915(b)(4) authority.

Provides personal care attendant, environmental accessibility adaptations, specialized medical equipment and supplies, and transition assistance services to individuals ages 65 or older, and individuals with physical or other disabilities ages 16-64 years who meet a nursing facility level of care.  

Provides day services – adult, in-home respite, prevocational services, supervised living, support coordination, supported employment, supported living, specialized medical supplies, therapy services, behavior support services, community respite, crisis intervention, crisis support, home and community supports, host home, in-home nursing respite, job discovery, shared supported living, and transition assistance services to individuals with autism, intellectual disabilities, or developmental disabilities ages 0 or older who meet an ICF/IID level of care.

Provides case management, respite, wraparound to individuals w/mental illness ages 18-21 and SED ages 0-21

Provides personal care attendant, respite, environmental accessibility adaptations, specialized medical equipment and supplies, and transition assistance services to individuals with physical disabilities or brain injury ages 0 or older who meet a nursing facility level of care.

WAIVER TERMINATED on 9/30/2014 – Provided case management, respite, and wraparound services to individuals with mental illness ages 18-21 and individuals with serious emotional disturbance ages 0-21 years who met a psychiatric residential treatment facility level of care.
