Provides care coordination, personal care, respite, financial management services, skills training, assistive technology, attendant care services, environmental modifications, home delivered meals, home health services, living well for better health (chronic disease self-management), matter of balance (falls prevention), non-medical transportation, and personal emergency response system services to individuals ages 65 or older and individuals with physical disabilities ages 18 or older who meet a hospital or nursing facility level of care.  This waiver operates with a concurrent 1915(b)(4) authority.

Provides care coordination, career planning, community support, home support (1/4 hour), home support-per diem, personal care, financial management services, individual goods and services, supports brokerage, work support, assistive technology, communication aids, consultation and assessment, employment specialist services, home accessibility adaptations, home support-remote support, non-traditional communication consultation, non-medical transportation, non-traditional communication assessments, occupational therapy (maintenance), physical therapy (maintenance), shared living (adult foster care), specialized medical equipment and supplies, and speech therapy (maintenance) services to individuals ages 65 or older and individuals with other disabilities ages 21-64 years who meet an ICF/IID level of care.  This waiver operates with a concurrent 1915(b)(4) authority.

Provides care coordination, career planning, home support (1/4 hour-level I), home support-per diem level II, financial management services, individual goods and services, supports brokerage, assistive technology, employment specialist services, home support- remote support, home support-per diem level III (increased neurobehavioral), non-medical transportation, work ordered day clubhouse, and work support-individual services to individuals with brain injury ages 18 or older who meet a nursing facility or ICF/IID level of care.  This waiver operates with a concurrent 1915(b)(4) authority.

Provides community support, home support (1/4 hour), per diem home support, work support-group, adult foster care/shared living, assistive technology, career planning, communication aids, consultation, counseling, crisis assessment, crisis intervention, employment specialist services, home accessibility adaptations, home support-remote support, home support-residential habilitation-family centered support, non-traditional communication consultation, non-medical transportation, non-traditional communication assessment, occupational therapy (maintenance), physical therapy (maintenance), specialized medical equipment and supplies, speech therapy (maintenance), and work support-individual services to individuals with autism or intellectual disabilities ages 18 or older who meet an ICF/IID level of care.  This waiver operates with a concurrent 1915(b)(4) authority.

Provides community support, home support 1/4 hour, respite, work support-group, financial management services, individual goods and services, supports brokerage, assistive technology, career planning, employment specialist services, home accessibility adaptations, home support-remote support, non-medical transportation, shared living, and work support-individual services to individuals with autism or intellectual disabilities ages 18 or older who meet an ICF/IID level of care.  This waiver operates with a concurrent 1915(b)(4) authority.

WAIVER TERMINATED on 12/27/2014 – Provided personal attendant, supports brokerage, financial management services, skills training, and personal emergency response system services to individuals ages 65 or older and individuals with physical disabilities ages 18-64 years who met a hospital or nursing facility level of care.
WAIVER TERMINATED on 3/27/2015 – Provided home support, respite, communication aids, consultation, home accessibility adaptations, and transportation services to individuals with autism or intellectual disabilities ages 5-21 years who met a hospital or ICF/IID level of care.  This waiver operated with a concurrent 1915(b)(4) authority.
