Provides adult day service, attendant care, care management, home and community assistance service, skilled respite, adult family care, assisted living, caregiver coaching, community transition, home delivered meals, home modification assessment, home modifications, integrated health care coordination, nutritional supplements, participant directed home care service, personal emergency response system, pest control, specialized medical equipment and supplies, structured family caregiving, transportation, and vehicle modifications services to individuals ages 65 or older and individuals with physical and other disabilities ages 60-64 years who meet nursing facility level of care.  This waiver operates with a concurrent 1915(b)(1) and 1915(b)(4) authority.

IN Traumatic Brain Injury Waiver (4197.R05.00)

Provides adult day services, attendant care, care management, home and community assistance, residential based habilitation, skilled respite, structured day program, supported employment, adult family care, assisted living, behavior management/behavior program and counseling, community transition, home delivered meals, home modifications, home modification assessment, integrated health care coordination, nutritional supplements, personal emergency response system, pest control, specialized medical equipment and supplies, structured family caregiving, transportation, and vehicle modification services to individuals with brain injury ages 0 or older who meet a nursing facility or ICF/IID level of care.
