DC Elderly and Persons with Physical Disabilities Waiver (0334.R05.00)

Provides adult day health, case management, homemaker, personal care aide, respite, assisted living, chore aide, community transition services, DSNP/managed care capitated waiver services, environment accessibility and adaptation services, individual directed goods and services, and participant-directed community support services to individuals ages 65 or older and individuals with physical disabilities ages 18-64 years who meet a nursing facility level of care.  This waiver operates with a concurrent 1915(a)(1)(a) and 1915(i) authority.

DC Individual and Family Support (IFS) Waiver (1766.R01.00)

Provides day habilitation, education supports, employment readiness, group supported employment, respite, personal care services, skilled nursing, assistive technology services, behavioral supports, companion services, creative arts therapies, in-home supports, individual supported employment, individualized day supports, occupational therapy, parenting supports, physical therapy, speech/hearing/language services, unpaid family/caregiver training, and wellness services to individuals with intellectual disabilities or developmental disabilities ages 18 or older who meet an ICF/IID level of care.

DC People with Intellectual and Developmental Disabilities (IDD) Waiver (0307.R05.00)

Provides day habilitation, employment readiness, in-home supports, residential habilitation, respite, supported employment, personal care services, skilled nursing, assistive technology services, behavioral supports, companion services, creative arts therapies, dental, family training, health assessment and coordination, host home, individualized day supports, occupational therapy, one-time transitional services, parenting supports, physical therapy, small group supported employment, speech/hearing and language services, supported living with transportation, supported living, and wellness services to individuals with intellectual disabilities or developmental disabilities ages 18 and older who meet an ICF/IID level of care.
