Provides homemaker, respite, adapted therapeutic recreational equipment and fees, assistive technology, community connector, hippotherapy, home accessibility adaptations, massage therapy, movement therapy, parent education, specialized medical equipment and supplies, vehicle modifications, youth day service for individuals with developmental disabilities ages 0 – 17.

Provides case management and in home support services for individuals who are medically fragile ages 0 – 17.

Provides adult day health, homemaker, personal care, respite, complementary and integrative health services- acupuncture, complementary and integrative health services- chiropractic, complementary and integrative health services- massage therapy, consumer directed attendant support services (CDASS), home delivered meals, home modification, in home support services (IHSS), life skills training, medication reminder, non-medical transportation, peer mentorship, personal emergency response systems (PERS), remote support technology, and transition setup services for individuals 65 to no maximum age and individuals with physical disabilities ages 18 to 64 years.

Provides day habilitation, prevocational services, residential habilitation, supported employment, dental services, vision services, behavioral services, home delivered meals, non-medical transportation, peer mentorship, specialized medical equipment and supplies, and transition setup for individuals with developmental disabilities ages 18 - no max age.

Provides adult day health, homemaker, personal care, respite, alternative care facility (AFC), consumer directed attendant support services (CDASS), home delivered meals, home modification, in home support services (IHSS), life skills training, non-medical transportation, peer mentorship, personal emergency response systems (PERS), remote support technology, supplies/equipment/medication reminder, and transition setup services for individuals aged 65 to no max age, individuals with physical disabilities ages 18 to 64, and individuals with HIV/AIDS age 18 to no max age.

Provides habilitation, respite, transition support services, community connector, hippotherapy, intensive support services, massage therapy, and movement therapy for individuals with developmental disabilities ages 0 – 20.

Provides respite care, bereavement counseling, palliative/supportive care services, expressive therapy, massage therapy, therapeutic life limiting illness support: individual counseling, family counseling, group counseling for individuals who are medically fragile ages 0-18.
Provides adult day service, homemaker, personal care, respite, alternative care facility (ACF), consumer directed attendant support services (CDASS), home delivered meals, home modifications, life skills training, non-medical transportation, peer mentorship, personal emergency response systems (PERS), remote support technology, specialized medical equipment and supplies, and transition setup services for individuals with mental illness age 18 to no maximum age.

WAIVER TERMINATED 12/30/13 - Provides adult day health, homemaker, personal care, consumer directed attendant support services, medication reminder, non-medical transportation, PERS for individuals w/HIV/AIDS ages 0 - no max age

Provides adult day health, day treatment, personal care, respite, behavioral management and education, consumer directed attendant support services, home delivered meals, home modification, independent living skills training, mental health counseling, non-medical transportation, peer mentorship, personal emergency response systems (PERS), remote support technology, specialized medical equipment and supplies/assistive devices, substance abuse counseling, supported living program, transition setup, and transitional living program services for individuals with brain injury age 16 to no maximum age.

Provides day habilitation, homemaker, personal care, prevocational services, respite, supported employment, dental services, vision services, assistive technology, behavioral services, health maintenance activities, hippotherapy, home accessibility adaptations, home delivered meals, life skills training, massage therapy, mentorship, movement therapy, non-medical transportation, peer mentorship, personal emergency response, recreational facility fees/passes, remote support technology, specialized medical equipment and supplies, transition setup, and vehicle modifications for individuals with developmental disabilities ages 18 to no maximum age.

WAIVER TERMINATED Effective 1/2/2018 – Provided behavioral therapy, ongoing treatment evaluation, and post service evaluation for children with autism ages 0 to 5 years.
