Provides day habilitation, homemaker, individual supported employment, personal care, prevocational services, respite, dental services, vision services, assistive technology, behavioral services, benefits planning, group supported employment, health maintenance activities, adaptive therapeutic equine activities, home accessibility adaptations, home delivered meals, life skills training, massage therapy, mentorship, movement therapy, non-medical transportation, peer mentorship, personal emergency response, recreational facility fees/passes, remote support, specialized medical equipment and supplies, transition setup, vehicle modification, and wellness education benefit services to individuals with developmental disabilities ages 18 or older who meet an ICF/IID level of care. This waiver operates with a concurrent 1915(b)(4) authority.
