Provides respite care, bereavement counseling, palliative/supportive care services, expressive therapy, massage therapy, therapeutic life limiting illness support: individual counseling, family counseling, group counseling, and wellness education benefit services to individuals with physical disabilities ages 0-18 and individuals who are medically fragile ages 0-18 years who meet a hospital or nursing facility level of care.  This waiver operates with a concurrent 1915(b)(4) authority.

Provides homemaker, respite, adapted therapeutic recreational equipment and fees, assistive technology, community connector, home accessibility adaptations, massage therapy, movement therapy, primary caregiver education, specialized medical equipment and supplies, vehicle modifications, wellness education benefit, and youth day services to individuals with developmental disabilities ages 0-17 years who meet an ICF/IID level of care. This waiver operates with a concurrent 1915(b)(4) authority.

Provides adult day health, respite, complementary and integrative health services- acupuncture, complementary and integrative health services- chiropractic, complementary and integrative health services- massage therapy, home modification, life skills training, non-medical transportation, peer mentorship, and wellness education benefit services to individuals ages 65 or older and individuals with physical disabilities ages 18-64 years who meet a hospital or nursing facility level of care. This waiver operates with a concurrent 1915(b)(4) authority.

Provides day habilitation, individual supported employment, prevocational services, residential habilitation, dental services, vision services, behavioral services, benefits planning, group supported employment, non-medical transportation, peer mentorship, specialized medical equipment and supplies, and wellness education benefit services to individuals with developmental disabilities ages 18 or older who meet an ICF/IID level of care. This waiver operates with a concurrent 1915(b)(4) authority.

Provides adult day health, respite, alternative care facility, home modification, life skills training, non-medical transportation, peer mentorship, and wellness education benefit services to individuals ages 65 or older, individuals with physical disabilities ages 18-64 years, and individuals with HIV/AIDS ages 18 or older who meet a nursing facility level of care. This waiver operates with a concurrent 1915(b)(4) authority.

Provides adult day service, mental health transitional living home service, respite, alternative care facility, home modifications, life skills training, non-medical transportation, peer mentorship, and wellness education benefit services to individuals with mental illness ages 18 or older who meet a nursing facility level of care. This waiver operates with a concurrent 1915(b)(4) authority.

Provides habilitation, respite, wraparound services, child and youth mentorship, community connector, massage therapy, movement therapy, and wellness education benefit services to individuals with developmental disabilities ages 0-20 and individuals with serious emotional disturbance ages 0-20 years who meet a hospital, nursing facility, or an ICF/IID level of care.  This waiver operates with a concurrent 1915(b)(4) authority.

WAIVER TERMINATED on 3/31/2014 – Provided adult day health, homemaker, personal care, consumer directed attendant support services, medication reminder, non-medical transportation, and personal emergency response systems services to individuals with HIV/AIDS ages 0 or older who met a hospital or nursing facility level of care.

Provides adult day health, day treatment, respite, behavioral management and education, home modification, independent living skills training, mental health counseling, non-medical transportation, peer mentorship, specialized medical equipment and supplies/assistive devices, substance abuse counseling, supported living program, transitional living program, and wellness education benefit services to individuals with brain injury ages 16 or older who meet a hospital or nursing facility level of care. This waiver operates with a concurrent 1915(b)(4) authority.

WAIVER TERMINATED on 7/1/2018 – Provided behavioral therapy, ongoing treatment evaluation, and post service evaluation to individuals with autism ages 0-5 years who met an ICF/IID level of care.

WAIVER TERMINATED on 6/30/2026 – Provided in-home support services and wellness education benefit services to individuals who were medically fragile ages 0-17 years who met a hospital or nursing facility level of care. This waiver operated with a concurrent 1915(b)(4) authority.
