Current Procedural Terminology (CPT®) codes copyright 2023 American Medical Association (AMA).  All rights reserved.  CPT is a trademark of the AMA.  No fee schedules, basic units, relative values, or related listings are included in CPT.  The AMA assumes no liability for the data contained herein.  Applicable Federal Acquisition Regulations (FAR) and Defense Federal Acquisition Regulation Supplement (DFAR-S) restrictions apply to government use.

The International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CMS) and the International Classification of Diseases, 10th Revision, Procedure Coding System (ICD-10-PCS) are published by the World Health Organization.  ICD-10-PCS is an official Health Insurance Portability and Accountability Act standard.

Healthcare Common Procedure Coding System (HCPCS) Level I codes and descriptors are maintained by the AMA. All rights reserved.

Healthcare Common Procedure Coding System (HCPCS) Level II codes and descriptors are approved and maintained jointly by the alphanumeric editorial panel (consisting of the Centers for Medicare & Medicaid Services, America's Health Insurance Plans, and Blue Cross and Blue Shield Association).

The Systematized Nomenclature of Medicine (SNOMED®) and SNOMED Clinical Terms (SNOMED CT®) are registered trademarks of International Health Terminology Standards Development Organisation.

The American Hospital Association holds a copyright to the Uniform Bill (UB) codes and uses of the Uniform Bill codes require a license from the American Hospital Association.  Anyone desiring to use the Uniform Bill codes in a commercial product to generate measure results, or for any other commercial use, must obtain a commercial use license directly from the American Hospital Association.  To inquire about licensing, please contact ub04@aha.org.
