Provides adult day health, day habilitation, homemaker, residential habilitation, respite, supported employment, attendant care, adult residential care, chore service, companion services, consultation, environmental accessibility adaptations, home delivered meals, non-medical transportation, personal emergency response system, skilled nursing, specialized medical equipment and supplies, and transition services for aged individuals ages 65 to no max age and individuals with physical and other disabilities ages 18 to 64 years.

Provides residential habilitation, respite, supported employment, financial management services, support broker services, adult day health, behavior consultation/crisis management, chore services, community support services (participant direction), environmental accessibility adaptations, home delivered meals, non-medical transportation, personal emergency response system, skilled nursing, specialized medical equipment and supplies for individuals w/autism, DD, IID ages 18 - no max age

WAIVER TERMINATED Effective 7/1/2016 – Provided family education, habilitative supports, respite, crisis intervention, family training, habilitative intervention, interdisciplinary training, and therapeutic consultation services for individuals with autism, intellectual disabilities, and developmental disabilities ages 3 to 6 years.
