Provides day habilitation, personal care, prevocational services, residential habilitation, respite, supported employment – individual, supported employment - small group, assistive technology, behavioral consultation, community transition, home or vehicle accessibility adaptations, nurse consultation, specialized medical equipment and supplies, and supported living services to individuals with autism or intellectual disabilities ages 12 or older who meet an ICF/IID level of care.  This waiver operates with a concurrent 1115 authority.
WAIVER TERMINATED on 3/30/2012 – Provided case management, personal care, respite, mental health services, and nutritional support services to individuals with HIV/AIDS ages 1 or older who met a hospital level of care.
WAIVER TERMINATED on 3/30/2012 – Provided adult day services, day habilitation, personal care, respite, assisted living, cognitive services, personal emergency response system, and specialized medical equipment and supplies to individuals ages 65 or older and individuals with physical disabilities ages 18-64 years who met a nursing facility level of care.
