The Value-Based Purchasing MAC Learning Collaborative, in place from 2011 – 2014, brought together officials from state Medicaid agencies and the Centers for Medicare & Medicaid Services to advance delivery system models that more effectively link payment to improved health care quality. To assist state Medicaid agencies facilitate the design and development of new integrated care models (ICMs), this Collaborative developed a concept development toolkit. The toolkit is intended to help states conceptualize and articulate key concepts for proposed ICM programs.
