This Amendment implements a Developmental Disabilities Health Home (DD Health Home) for individuals statewide served through the Missouri Department of Mental Health, Division of Developmental Disabilities (DD), who have a qualifying chronic health condition, have or are at risk of developing another condition, and are eligible for Division of DD services.
