CHIP State Plan Amendments

As part of a strategy to increase transparency, enhance efficiency, and reduce burden for states in the review and approval process of state plan amendments (SPA), CMS committed to providing a tool kit to assist states in the development of complete submissions, and facilitate quicker adjudication of SPA submissions. This page provides a collection of existing CHIP SPA templates for completing the templates that will be enhanced as more tools are developed.

States apply for changes to their CHIP state plan using the CHIP State Plan Template.  Persons with disabilities having problems accessing the PDF files may call 410-786-0429 for assistance. To request a Word version of the CHIP State Plan, please contact your CHIP Project Officer.

CHIP State Program Information

In order to be eligible for payment under this statute, each State must submit a Title XXI plan for approval by the Secretary that details how the State intends to use the funds and fulfill other requirements under the law and regulations at 42 CFR Part 457.

States can design their CHIP program in one of three ways:

1.    Separate CHIP: a program under which a state receives federal funding to provide child health assistance to uninsured, low-income children that meets the requirements of section 2103 of the Social Security Act.

2.    Medicaid expansion CHIP: a program under which a state receives federal funding to expand Medicaid eligibility to optional targeted low-income children that meets the requirements of section 2103 of the Social Security Act.

3.    Both Separate CHIP and Medicaid expansion: a program under which a state receives federal funding to implement both a Medicaid expansion and a separate CHIP.

Maine
This SPA allows CHIP eligibility for all children with access to public employee coverage on the basis of a family member's employment who are otherwise eligible targeted low-income children as defined at 42 CFP* 457 .310, provided that the child's household income meets the same income standards as those applied to targeted low-income children under the CHIP state plan.

North Dakota
The approval of SPA ND-13-0103 includes full approval of the state's alternative single streamlined paper application only. The state is using an interim alternative paper application formultiple human services programs. By March l, 2018, the state will implement a revised alternative paper application used to apply for multiple human services programs that addresses CMS concerns outlined in the companion letter issued with this SPA approval.

Florida
Through this SPA, the state provides temporary adjustments to the state's enrollment and redetermination policies and cost-sharing requirements for children in families impacted by disaster events.

Georgia
Through this SPA, Georgia adds nonemergency medical transportation, case management services, and enabling services as covered benefits under the CHIP state plan.

California
This SPA updates California's full state plan by removing the former Healthy Families component of the state's separate CHIP, which covered children up to age 19 above the Medicaid levels with income up to 250 percent of the Federal poverty level (FPL).

Ohio

This SPA permits Ohio to implement a health services initiative (HSI) to provide lead abatement services in the homes of low-income children and pregnant women and to establish an online lead-safe housing registry.

New York

This SPA describes NY\u2019s policy to provide retroactive coverage for newborns if the application is received within 60 days after birth, and implements two CHIP Health Services Initiatives (HSI) by providing funding to: 1) support the training of school officials and staff to administer naloxone rescue kits to reverse opioid overdoses, and2) support the provision of emergency meals and nutrition education to low-income children and their families.
