The purpose of this SPA is to amend the provisions governing payments for well baby and transplant services, within the inpatient hospital services program, in order to align with current practices.

This amendment covers adult vaccinations and their administration without cost sharing.

This plan amendment updates the fee schedule rates for all AZ non-institutional services, other than Outpatient hospital services, paid via fee schedule.

This plan amendment updates Emergency Medical Transport (EMT) rates.

This plan amendment updates the fee schedule rates for outpatient hospital services.

This amendment is to update the State Plan to include Collaborative Care Model (CoCM) services, as directed by Senate Bill 672, 87^th^ Legislature, Regular Session, 2021.

This SPA amends the State Plan to add coverage of select weight loss medications.

Updates the description of Peer Support Services and revises the qualifications and licensing requirements for Certified Peer Specialists.

Adds an assurance that the Department of Medical Assistance Services will make coverage and billing code modifications when the Advisory Committee on Immunization Practices (ACIP) and/or U.S. Preventive Services Task Force (USPSTF) “A” and “B” recommendations change. The amendment also makes a technical change to revise reference to section “4016” of the Patient Protection and Affordable Care Act to section “4106.”
