RULE-2808	If the provider middle initial is populated on the provider attributes main file, then the provider first name reported must not contain any pipe or asterisk characters.
RULE-2809	If the facility group individual code is not equal to '03' on a provider attributes main segment from a PRV file, then the value for provider middle initial must be null or invalid.
