The Centers for Medicare & Medicaid Services (CMS), as part of the Testing Experience and Functional Tools (TEFT) demonstration, works in partnership with the Office of the National Coordinator for Health Information Technology (ONC) to identify and harmonize an electronic long term services and supports (eLTSS) service plan standard to enable electronic exchange of information across home and community-based service settings and organizations to improve care coordination. TEFT grantees facilitated two rounds of pilots to exchange the eLTSS dataset in conjunction with ONC’s Standards and Interoperability (S&I) Framework.
