Work with Communities to Promote Best Practices of Healthy Living

The Centers for Medicare and Medicaid Services organizes measures in the state health system performance pillar of the Scorecard using the domains defined in the Meaningful Measures initiative. The initiative identifies the highest priority areas for quality measurement and improvement. In the future, the Scorecard will include measures that focus on promoting best practices of healthy living in partnership with communities, which supports equity in care for all Medicaid and Children's Health Insurance Program (CHIP) beneficiaries. Implementing community-level best practices is important for beneficiaries with long-term care needs and those with social risk factors.
