Medicaid Exception to Recovery Audit Contractor (RAC) Program

This plan amendment incorporates the Montana legislatively approved provider rate increase and updates the date of the fee schedule for state plan services on its Introduction Page.  

This plan makes changes to the reimbursement methodology for case management services for adults with chronic illnesses and disabilities from a daily rate to a monthly rate. 

This SPA describes the methodology used by the state for determining the appropriate FMAP rates, including the increased FMAP rates, available under the provisions of the Affordable Care Act applicable for the medical assistance expenditures under the Medicaid program associated with enrollees in the new adult group adopted by the state and described in 42 CFR 435.119. 

This plan amendment increases the Targeted Case Management (TCM) State Plan Services rate and conversion factor to reflect the 2021 legislative appropriation of a 1% increase. 

This SPA amends the Medicaid State Plan to implement specified home health rate increases. Specifically First, this SPA increases rates by 6% for codes Tl004 and Tl021 for home health aide or certified nursing assistant services provided by licensed home health agencies.

This plan implements a supplemental payment for the difference between payment at Medicare and Medicaid rates for physician services provided by the physician group affiliated with Connecticut Children’s Medical Center (CCMC). 

This plan amendment updates the date of the fee schedule to implement a revised fee schedule, dated January 1, 2021, which reflects an updated reimbursement rate for Home Support Services (HSS).  The revised fee schedule  also reflects an updated reimbursement rate for Comprehensive School and Community Treatment (CSCT). 

This plan amendment allows Connecticut to increase emergency and non-emergency ambulance rates by 10% (excluding the mileage rate) and increase the ambulance mileage rates for all emergency and non-emergency transports by $3.00.
