Managed Care - Adult Group Moves from MCO to QHPs, except Medically Frail.

This State plan amendment (SPA) updates Attachment 4.19-D to remove the methodology for calculating the three behaviorally complex add-on tier rates and adds the actual calculated rates to the plan page; links the tier definition to the state Medicaid service manual as it reads on January 28th, 2016; and removes the odd-year wage update.

Clinical Laboratory Fee Schedule.

Chronic Disease and Rehabilitation Outpatient Hospital Services.

This Amendment adopts provisions in the Medicaid Program to expand Medicaid coverage to the new adult group pursuant to the provisions of the Affordable Care Act.

Psychosocial Rehabilitative services.

The purpose of this State Plan Amendment is to submit a corrected organizational chart. The chart submitted with MA-15-005 contained an error.

Provides for a per diem increase of two dollars and nine cents ($2.09) for nursing facilities and HIV nursing facility services.

Coverage & Limitations and Payments Other Licensed Practitioner Services.

Revises temporary Vital Access Provider/Safety Net Provider (VAP/SNP) enhanced payments for eight nursing homes and adds new payments for 47 additional facilities.
