The purpose of this new state plan is to add a target group for Targeted Case Management Services for Youth with Serious Emotional Disturbance( SED) in an Out of State Psychiatric Residential Treatment Facility (PRTF).

Incorporates the MAGI-Based Eligibility Groups SPA into Pennsylvania's State Plan in accordance with the Affordable Care Act.

Specifies options for presumptive eligibility conducted by hospitals.

This SPA expands the state's health homes program statewide and adds Hepatitis C as a covered chronic condition. The State plan pages for this SPA were submitted through the Medicaid Model Data Lab (MMDL) on September 17, 2014.
