This amendment seeks to eliminate Targeted Case Management services for Individuals with HIV, Pregnant Woman, Persons Identified as Seriously Mentally Ill or Seriously Emotionally Disturbed, Low Functioning Severely and Persistently Mentally Ill Adults Needing Assertive Community Treatment (ACT), Individuals with Developmental Disabilities, and Case Management for. Elderly or Disabled Individuals Diverted/Deinstitutionalized from Nursing Facilities from the State Plan.

IN-11-014

The attached pages offer affirmation that the State of Indiana does not prohibit curative treatment upon the election of the hospice benefit by or on behalf of a Medicaid or CHIP eligible child.

IN-11-016

This amendment imposes a 5% rate reduction on all medical supplies provided from July 1, 2011 to June 30, 2013. This SPA also seeks to change the methodology of Medical Supply reimbursement to utilize Medicare rates as the reimbursed amount when available, or the acquisition cost when Medicare rates are not available.

This amendment seeks to revise the reimbursement methodology of DM Medical Equipment and Hearing Aids as well as reduce the rates by 5% for services rendered between July 1, 2011 and June 30, 2011.
