This amendment limits eyeglass frames for adults twenty-one (21) years of age and older to one pair every two years and lenses to one pair every three hundred and sixty five days.

This State Plan Amendment addresses the new requirements regarding Estate Recovery under section 1917(b)(1) of the Social Security Act by encouraging dual eligible beneficiaries to fully utilize Medicare cost-sharing benefits available through the Medicare Savings Program.

This SPA was initially submitted on September 23, 2011 with a stated purpose of amending the Title XIX Outpatient Hospital Reimbursement Plan payment methodology, effective July 1, 2011. Based on the HCFA 179 submitted by the State, Federal budget impact would be $23,043,000 in FFY 2010-11 and $69,128,000 in FFY 2011-12.

FL-11-015

This SPA proposed to update prior authorization requirements for inpatient and outpatient hospital services to reflect current policy. It also aligned Attachment 3.1-B (Medically Needy) with Attachment 3.1-A (Categorically Needy).

FL-11-016

This amendment allows self-direction for the less than 21 years old population eligible to self-direct their State Plan personal care services.

This amendment revises Florida's Tribal consultation process, which consists of Florida Medicaid sending written notices to the Miccosukee Tribe of Florida and the Seminole Tribe of Florida, if the change is expected to have a direct impact upon the Tribes.

This SPA was initially submitted on September 24, 2012 with a stated purpose of amending the Florida County Health Department Reimbursement Plan payment methodology, effective July 1, 2012. Based on the HCFA 179 submitted by the State, federal budget impact would be $3,983,000 in FFY 2011-12 and $15,993,000 in FFY 2012-13.

GA-09-003

The amendment updates the maximum allowable reimbursement for durable medical equipment services to more accurately match payments to the level of funding that was made available in the State Fiscal Year 2010 Appropriations Act.

GA-10-005

This amendment was submitted to implement the Asset Verification System that will be used to electronically verify the assets of aged, blind or disabled Medicaid applicants and recipients through access to information held by financial institutions.
