Proposes the implementation of Phase II counties for the State's Health Homes initiative.

This SPA will expand targeted case management services to Onondaga County to permit a nurse, who does not have a Bachelor's of Science in Nursing, but who is bilingual and enrolled in a Bachelor's degree program in nursing, to serve as a case manager to an underserved population.

This amendment proposes that the capital cost component of the rate for eligible residential health care facilities shall be adjusted to reflect Medicaid's share of the costs of the annual debt service related to the financing of an automatic sprinkler system that will be in compliance with new federal regulations.

The State of New York is electing, as of May 1, 2012, the option to rely on a finding from an Express Lane agency (CHPlus) when determining whether a child satisfies one or more components of Medicaid eligibility. (Children's Health Insurance Program Reauthorization Act of 2009).

New York State proposes, as of November 1, 2011, to disregard the difference between 100% and 133% of the Federal Poverty Level for children who have attained 6 years of age but have not attained 19 years of age.

Implements Trend Factor No Greater Than Zero Percent for Non-Institutional Services.

This SPA updates the wage equalization factor component of the service rates for the early intervention program and reduces the rates by five percent for these services on and after May 1, 2011.

This amendment implements regulations for Provider Preventable Conditions (PPCs) and related payment adjustments for Medicaid and incorporates the CMS pre-print for PPCs for outpatient services into Attachment B. New York implemented the CMS pre-print for inpatient services in Attachment A under TN 11-046-A, approved on May 25, 2012.

Imposes Utilization Thresholds for Certain Clinic Services Related to Behavioral Health and Reduced Provider Reimbursement for Visits Which Exceed Those Thresholds.

The spa proposes to continue cost savings measures for certain non-institutional services for the period April 1, 2011 to March 31, 2013.
