Functional Assessment Standardized Items

The Centers for Medicare & Medicaid Services (CMS), as part of the Testing Experience and Functional Tools (TEFT) demonstration, tested the use of the Functional Assessment Standardized Items (FASI) measures among individuals receiving home and community-based services (HCBS), aligning with national efforts to create exchangeable data across Medicare and Medicaid programs.

FASI includes a new set of functional assessment items, which originated from the Continuity Assessment Record and Evaluation tool created for post-acute care settings. TEFT states field tested two rounds of this modified set of functional assessment items for use with beneficiaries of HCBS. Currently, the FASI items are being integrated into the CMS Data Element Library, which serves as a repository for all standardized items used in CMS assessments and their associated health information technology standards.
