Learn How to Report the CMS 416 Dental Data
Welcome to the training modules on Form CMS-416 Dental Data Reporting, produced as part of the Centers for Medicare & Medicaid Services’ (CMS) Oral Health Initiative (OHI). In 2010, CMS launched the OHI with specific national and state goals to increase the use of preventive dental services by children enrolled in Medicaid and the Children’s Health Insurance Program (CHIP). The data collected on the Form CMS-416 tracks levels of utilization in Early and Periodic Screening, Diagnostic, and Treatment (EPSDT), the benefit for children enrolled in Medicaid, and enables states and CMS to track progress toward OHI goals.
The training modules are designed to assist state Medicaid and CHIP staff, and their contractors, to improve the quality of the dental data they report on the Form CMS-416. This website can be used to navigate to the training module YouTube videos and access resources related to Form CMS-416 reporting.
