The U.S. Administration for Community Living (ACL), which includes the Administration on Aging, sponsors this website to make information and resources available to states and community organizations. This ACL website provides a forum to allow a diverse community of stakeholders involved in making changes to their long-term care system to exchange ideas, knowledge, and best practices.

Aging and Disability Resource Centers Program / No Wrong Door System

The U.S. Administration for Community Living (ACL) website offers information about ACL and CMS funded initiatives aimed at improving the coordination of care transitions through an Aging & Disability Resource Centers (ADRC) Program / No Wrong Door (NWD) System. These programs support state efforts to streamline access to long-term services and support (LTSS) options for older adults and individuals with disabilities. Aging Network Care Transitions Toolkit

The Care Transition Toolkit, targets organizations that are interested in learning more about how to prepare for a role in care transitions programs. The tools and resources at this site can assist organizations in formalizing their efforts for future funding and program opportunities. This toolkit includes lessons learned from States that received funding from the Aging and Disability Resource Center (ADRC) Program, ADRC Evidence-Based Care Transitions Program, Person-Centered Hospital Discharge Model, Community Living Program (CLP), and the Veteran Directed Home and Community Based Services (VD-HCBS) Program.

The Clearinghouse for Home and Community-Based Services

Hosted by ADvancing States, HCBS.org promotes the development and expansion of home and community-based services by gathering resources and tools for research, policymaking and program development into this one-stop website. The information shared on this website helps professionals be more effective in supporting real choice and independence for older Americans and people with disabilities

Balancing Incentive Program

The former Balancing Incentive Program provided financial incentives to States to increase access to non-institutional long term services and supports (LTSS) in keeping with the integration mandate of the Americans with Disabilities Act (ADA), as required by the Olmstead decision and was created by the Affordable Care Act of 2010 (Section 10202). Other CMS Initiatives to Support Community Living

CMS is working in partnership with States, consumers and advocates, providers and other stakeholders to create a sustainable, person-driven long-term services and supports system in which people with disabilities and chronic conditions have choice, control and access to a full array of quality services that assure optimal outcomes, such as independence, health and quality of life. Evidence-based Disease and Disability Prevention Program (EBDDP)

This U.S. Administration on Aging website on the EBDDP Program provides information, links and go-to resources to states and community organizations on the issues of health and prevention programs aimed at empowering older adults to take control of their health.

Housing Capacity Building Initiative for Community Living

This initiative was implemented through the U.S. Department of Housing and Urban Development (HUD) and U.S. Department of Health and Human Services (HHS), under the Centers for Medicare & Medicaid Services (CMS) Money Follows the Person (MFP) Rebalancing Demonstration, to assist federal, state and local agencies and community partners to create a sustainable, collaborative system between housing and human services agencies. Tools and resources are provided to help States improve transition outcomes for individuals with disabilities seeking community living.

Mathematica Policy Research – Money Follows the Person National Evaluation

Money Follows the Person (MFP) evaluation reports assess how state long-term care systems change to support the transition of people from institutions to the community, whether the changes were successful and sustainable, and to what extent MFP helps balance state long-term care spending. The evaluation also analyzes the effects of MFP on Medicaid beneficiaries’ health and quality of life, as well as identifying characteristics of individuals and state programs strongly associated with success.

National Long Term Care Ombudsman Resource Center (NORC)

The National Long-Term Care Ombudsman Resource Center provides support, technical assistance and training to State Long-Term Care Ombudsman Programs and their statewide networks of almost 600 regional and local programs. The Center's objectives are to enhance the skills, knowledge, and management capacity of the State programs to enable them to handle residents' complaints, represent resident interests, and engage in both individual and systemic advocacy.

Testing Experience and Functional Tools (TEFT)

In March 2014, CMS awarded TEFT grants to nine states to test quality measurement tools and demonstrate e-health in Medicaid community-based long term services and supports (CB-LTSS). The grant program, spanning four years through March 2018, was designed to field test an experience of care survey and a set of functional assessment items, demonstrate personal health records, and create a standard electronic LTSS plan.

Workforce Initiative

As the primary funder of long-term services and supports under the Medicaid Program, CMS maintains a vital interest in both the paid and informal direct service community workforce. Through the Workforce Initiative, CMS provided state Medicaid agencies, researchers, policymakers, employers, consumers, direct service professionals, and other state-level government agencies and organizations easy access to information and resources they may need about the direct service workforce. These resources are applicable to the entire range of consumer populations supported by direct service workforce.
